Shaw Exercise Physiology is accredited to provide services through the following funding bodies:
National Disability Insurance Scheme (NDIS)
Our services are available through NDIS under the following support categories:
- Capacity Building – Improved Health and Wellbeing
- Physical Wellbeing Activities
- Advice provided by an Exercise Physiologist regarding the exercise required
- Physical Wellbeing Activities
- Therapy Supports
- Assessment Recommendation Therapy or Training – Exercise Physiologist
The number and frequency of the program sessions will be determined in your initial consultation in conjunction with your goals and needs.
Medicare
Our services are available through Medicare via a Chronic Disease Management Plan written by your GP. You may access up to a maximum of 5 program sessions funded under this scheme per calendar year.
WorkCover Queensland
To access our services through WorkCover Queensland, you must have a work-related injury and a Work Capacity Certificate completed by your GP, which evaluates your rehabilitation requirements. A referral form will then need to be completed by your GP.
Department of Veterans Affairs (DVA)
Our services are available via a DVA referral form (D0904) completed by your GP. Each treatment cycle is for up to 12 program sessions in total, including an initial consultation. Ongoing treatment cycles are on a clinical needs basis and will be discussed with you during your program.
Compulsory Third Party (CTP) Insurance
You can access our services through CTP by obtaining a referral from your GP or CTP insurer directly. If your referral is from your GP, we will need to gain approval from your insurer before commencing services.
Private Health Insurance
We are accredited to provide services through all private health insurance funds. You can contact us directly to book your initial consultation, as no referral form is required. The number of program sessions available to you is based off your private health funds rebate amount and funding limits available. If your funding limit is reached for the calendar year, you are welcome to continue accessing our services through self-funding until your benefits roll over for the next calendar year.

Initial Consultation: 60 minutes | $140
Subsequent Sessions: 45 minutes/session | $105
Self-funding
You can contact us directly to book your initial consultation, as no referral form is required. The number and frequency of the program sessions will be determined in your initial consultation in conjunction with your goals and needs.
